FEES
Fees are calculated based on the schedule below. Services may be provided in-person, via video conference, via telephone or via email. No fees will be charged for travel or preparation unless permission is received
from the client; however, time for meeting on behalf of the student, scoring assessments, and creating and monitoring academic or executive functions intervention plans is billable. If a session is extended because of late pick-up or mutual consent between both parties, the hourly rate is applicable for that time.

Academic Tutoring
-Grades 5 & below: $90/hr Individual or $110/hr Duo
-Grades 6 & above: $100/hr Individual or $120/hr Duo

Integrative Academic & Executive Functions/Self-Regulation Tutoring
-Grades 5 & below: $95/hr Individual or $115/hr Duo
-Grades 6 & above: $105/hr Individual or $125/hr Duo

Other Services
-Executive Functions Coaching: $130/hr Individual
-Educational Evaluations: $110/hr Individual
-Specialty Reading Instruction: $110/hr Individual
-Private Academic/Enrichment Class: $90/hr for 1 student, $55/hr/student for 2 students, $45/hr/student for 3 students, $35/hr/student for 4 students, $30/hr/student for 5 or more students; a small supply list may be required
-Private Executive Functions Class: $130/hr for 1 student, $80/hr/student for 2 students, $65/hr/student for 3 students, $55/hr/student for 4 students, $50/hr/student for 5 or more students; a small supply list may be required

PAYMENT
Monthly statements will be sent out electronically by the 3rd business day of each month for all hours incurred during the prior month. Prior to the start of services, the family is required to set up an account at peakacademics.teachworks.com and to enter credit card information for monthly autopay via Stripe, which will occur between the 5th and 8th day of the month following the month in which services were rendered. Alternate payment schedules may be arranged by contacting Peak Academics. All credit card information will be stored through a PCI compliant system.

To use the online payment service, the family will need to create an account. When creating an account, the family must provide accurate and complete information. The family is solely responsible for the activity that occurs on the account, whether expressly authorized by the family or not. The family is responsible for keeping the account password secure. The family may change the password at any time by updating the account profile. In addition, the family agrees to immediately notify Peak Academics of any unauthorized use of its password or account. Peak Academics will not be liable for any loss or damage arising from
or related to a failure to secure the account and password.

Peak Academics is not liable for any actions by Stripe or any other third-party vendor processing payments, or any interaction between you and the third-party vendor.

Peak Academics reserves the right to suspend services and/or cancel this agreement if payments are not made on time.

CANCELLATION & REFUND
For individual and duo services, sessions are scheduled as a weekly recurring appointment until the family provides written notice to stop or suspend services. The family may cancel an appointment for an individual or duo service up to 24 hours before a scheduled time by contacting the individual tutor, instructor or evaluator directly. Missed sessions without 24-hour notice will be charged at the full rate. If there are three or more cancellations from a family within a period of three months, the agreement may be cancelled.

For private group classes for three or more students, the family schedules and agrees via email to pay for a specific number of scheduled classes prior to the class' initiation. Missed classes will be charged at the full rate. There are no makeups.

Peak Academics may cancel scheduled services if a conflict, such as instructor illness, arises. Peak Academics will attempt to provide a substitute instructor or offer make-up services. If neither of these options is feasible, the scheduled session or class will be cancelled and the family will not be charged.

LATE ARRIVAL & CHANGE OF SCHEDULE
There will be no adjustment to fees based upon late arrival or early departure by the student. The tutor, instructor or evaluator agrees to wait fifteen minutes from the time of the scheduled session for the student's arrival before leaving and considering that session cancelled without sufficient prior notice and therefore subject to the terms set forth in the CANCELLATION AND REFUND policy.

NO WARRANTIES
Although research indicates that students who obtain support for homework, test preparation, and other skill support may show significant improvement in their performance, we make no promises or warranties with regards to the result of our services.

SUPERVISION
If the student is under the age of 18, a guardian (or an adult approved by a guardian) must be on the premises during in-home sessions. If no adult is present, then the session will be considered cancelled. Discuss alternate arrangements, such as a public meeting place, if necessary.

RELEASE
Peak Academics may use images, videos, or reproductions featuring the student’s work free of charge for promotional, advertising or other lawful purposes; provided such use does not feature the student’s name or the student’s body.

WAIVER OF LIABILITY
Peak Academics, LLC, its managers, members, owners and independent contractors are released from liability of any accident, illness, injury, or death or damage to personal property associated with activities during services.
